(2) What to bring for an outpatient visit
- Hospital card issued by Chutoen General Medical Center (if you have one)
- Referral letter
- Health insurance card
- Medication record book (OKUSURI TECHŌ) or medication you are taking (or its Information for Use)
- Maternity handbook (BOSHI- TECHŌ) if the patient is an infant

- The doctor at your clinic will make an appointment with Chutoen General Medical Center for you
- If you make your own appointment, please come to Counter No. 8 “Regional Medical Support Center” on the 1st floor, or call 0537-28-8021.(Japanese only)
- Please come to Counter No. 8 “Regional Medical Support Center” on the 1st floor to check in.
④ Pay your bill
(4) If you don’t have a referral letter:
※ An additional ¥7,700 (including tax) will be charged as the Special First-Consultation Fee (TOKUTEI SHOSHIN-RYŌ).

- Some departments require a referral letter for all first-consultation visits. In some cases, you may not be able to see a doctor depending on conditions.

(5) Revisits (with an appointment)
① Check in at the automated check-in machine at No. 5 Automated Revisit Counter on the 1st floor.

(6) About medication
Prescriptions must be filled at an external pharmacy. Please take your prescription to the pharmacy of your choice.
For your convenience, you can fax your prescription to a pharmacy at the External Prescription Fax Service counter. To use the fax service, first pay your bill, go to the Fax Service counter, and show your prescription along with the receipt of your payment to the staff. The staff will fax your prescription to the pharmacy of your choosing.

Expiration date of prescriptions
Please take your prescriptions to the pharmacy within four (4) days (including the date of issue.)
In-hospital prescription
Some special medication is dispensed only at the hospital dispensary. If your doctor issues in-hospital prescription, please receive a dispensary order (OKUSURI HIKIKAE-KEN) and take it to the hospital dispensary. A pharmacist will consult with you on how to take the medication before dispensing it.
(7) Outpatient visits to the emergency room
In cooperation with clinics and medical institutions in the local community, Chutoen General Medical Center provides for urgent medical needs of patients in acute conditions requiring emergency hospitalization and/or surgery. The Emergency Medical Center (emergency room) is open 24 hours a day every day including holidays and at night.

The emergency room nurse assesses the urgency and seriousness of the condition of each patient and determines the order of consultation.
If your condition is determined to be less urgent than that of another patient, you may be asked to wait at the emergency room for a long time before your consultation. This is to save the lives of patients with more urgent medical needs. We appreciate your understanding.
The order of consultation may change if the conditions/symptoms change after the initial assessment.

Before visiting the emergency room for outpatient consultation…
- Call the hospital first. (Japanese only)
Phone: 0537-21-5555 (Please tell the operator that you want to visit the emergency room) - The emergency room staff will take patient information over the phone.
The staff will take the basic patient information, such as name, birth date, gender, symptoms, etc. and the nurse will assess the urgency of the case. The staff may recommend that you visit another medical care provider depending on the situation at the Emergency Medical Center and/or the patient’s symptoms. - What to bring
