Hospital Admission



(1) Procedure for hospital admission

① Please complete the forms below and submit them to the Admission and Discharge Reception desk:

  • Hospital admission application form (NYŪIN MŌSHIKOMISHO/ NYŪIN SEIYAKUSHO)
  • Inpatient intake form (KANJA MONSHIN-HYŌ)
  • List of consent forms (DŌI ICHIRAN/ DŌISHO)
  • Consent form for scheduled test/ surgical procedure
  • Food allergy intake form         
  • Consent for use of specimen for the purpose of academic papers and medical education
  • Certificates (certificate of discharge from other medical institution, nursing care plan, etc. if applicable)

② Please show the following items at the Admission and Discharge Reception desk:

  • Hospital card
  • Health insurance card  
  • Eligibility Certificate for Ceiling-Amount Application (GENDOGAKU TEKIYOU NINTEI-SHŌ) (if applicable)
  • ICertificate of subsidy recipient (NHI Recipient Card Holder for Seniors [KŌREISHA JUKYŪSHA-SHŌ], Eligibility Certificate for Children’s Medical Cost Subsidy [KODOMO IRYŌ-HI JUKYŪSHA-SHŌ], etc.)

(2) What to bring

  • Medication record book (OKUSURI TECHŌ)
  • Prescribed medication including eye drops, topical ointment, injections, etc., if any
  • Toiletries (tooth paste/ brush, plastic cup to use when brushing teeth, etc.)
  • Essential personal items/Underwear/ towels/ bath towels/ tissue paper/ shampoo and conditioner/ soap
    ①Undershirt which opens in front if you need help in changing clothes
    ②Bring 2 – 3 sets of pajamas if you bring your own
  • Indoor shoes (ones with soft soles; no slippers allowed for safety)
  • If using dentures: Denture case/ cleaning tablets/ denture adhesive, etc.
  • If using diapers: Diapers/ wet-naps ※Diapers may be provided for a per-day fee for your convenience
  • If you need to do laundry: Laundry detergent (laundry washers and dryers are available)
  • Baby sling (if the patient is a child admitted to the pediatric ward and if available)
  • Family seal (INKAN) (if required)
  • Earphone (required when watching TV)
  • Flashlight
  • Electric razor (for male patients)
※ For safety, please do not keep valuables or a large sum of cash in your hospital room.
※ Please put your name on all items.
※ For safety, no sharp object (box cutter, scissors, razors) or electric kettle is allowed.

(3) Please observe the following rules during your stay at Chutoen General Medical Center

① To prevent theft:

Please keep your valuables to a minimum. If you witness a suspicious person or object, please notify the hospital staff immediately.

② To prevent infections:

Hand washing and gargling are crucial to prevent infections.
Please clean your hands with running water and/or alcohol found in hallway dispensers. Also please wear a surgical mask.

③In case of disaster:

  • Please check the evacuation route and the nearest emergency exit when you are first admitted to the hospital.
  • In case of disaster, please stay calm and follow the instructions of hospital staff.
  • Never use an elevator.
  • In case of fire, keep away from the source of the fire.

④ No alcohol, no smoking

No alcohol or smoking allowed in hospital premises.

⑤ Parking

Please avoid parking your car in the hospital parking lot during your hospital stay.
If you are admitted to the hospital in an emergency case or if you had no choice but to park your car in the parking lot, please ask your family member or friend to remove it from the parking lot. Thank you for your cooperation.

⑥ For patient safety

All inpatients wear a name band on their wrist or ankle while admitted to the hospital. The staff also ask your name when they need to double check your identity before providing the prescribed treatment or procedure.

⑦ Other rules to observe

  • In principle, doctors can meet a patients’ family members on weekdays before 5:00 pm to explain about tests and surgical procedures.
  • Please be mindful not to damage hospital property.
  • Please refrain from acts and behaviors which may disturb other patients, such as talking loudly, entering other patients’ rooms without permission.
  • Abusive language and behaviors as well as sexual harassment by patients are disruptive to providing proper patient care and treatment. Patients demonstrating such disruptive behaviors may be asked to leave the hospital. The hospital may call the police in serious cases.
  • No postal mail to patients are accepted by the hospital.

⑧ Consultation

A full-time medical social worker is stationed at the hospital. If you and your family members have any concerns after discharge from the hospital, we recommend that you consult with the social worker as soon as possible.
If you wish to receive a second opinion from another medical institution, please consult with the medical social worker. A referral letter from the Chutoen General Medical Center will be required to seek consultation at another hospital.

(4) During your stay at the hospital

Meals

Meals are prepared as instructed by your doctor. Please consume only the meals prepared for you during your hospital stay. A meal may not be served to you if your scheduled tests or symptoms require fasting.

〈Serving meals and collecting dishes and plates after meals〉
Meals are distributed in a temperature-controlled cart to be served at an appropriate temperature. Empty dishes and plates are collected one hour after the food is served.
〈Food allergy〉
The menu is changed accordingly if you have a food allergy. Please complete the food allergy intake form and give it to the nurse.

〈Breakfast options〉
You can choose rice or bread for breakfast. Please tell your nurse by 10:00 the day before if you want bread with your breakfast the next morning. Bread option may not be available depending on the menu.
〈Options in menu items〉
Some menu items may be changed upon request if you have food restrictions. Please ask the staff.
※ Only the main dish may be changed if the request is solely based on personal preference.

Press the nearest buzzer if you need to call a nurse.

Shower room open hours

7:00 am – 5:00 pm (Doctor’s permission is required to shower.)
※ Please tell your nurse before using the shower room.

Changing linens

Linens are changed once a week on Tuesday and as needed.

Changing rental hospital gown/ pajamas

Hospital gown/pajamas are changed twice a week on Monday and Thursday and as needed.

Short leave from the hospital

With your doctor’s permission, you may leave the hospital for a short time of up to two days and one night. Be sure to ask your doctor or nurse if you wish to leave the hospital for a short time. (You may need the doctor’s permission to leave your hospital ward.)

In-room amenities

  • Each hospital bed is equipped with a TV, a refrigerator, a dresser with a safe, and closet space.
  • The pay TV and refrigerator are available in a standard patient room. Please buy a TV card to operate the TV and the refrigerator.
  • Please use an earphone to listen to TV or radio unless you are in a private room.
  • TV cards are available for purchase from the machine in the common area on each floor. Earphones are available for purchase at the convenience store.

Coin-operated laundry equipment

Coin-operated clothes washer and dryer are available for use on each floor in the West Ward between 8:00 am and 8:00 pm

(5) Charges for your hospital stay

Charges for your hospital stay are calculated and billed according to the applicable health insurance rates. The items not covered by insurance are charged according to the billing policy of Chutoen General Medical Center.

※ Private room and gown/pajama rentals are tax-free if the patient is admitted for childbirth.

How to pay

The bill for your inpatient charges is delivered to your room prior to the schedule discharge date. Please pay the bill on the day of discharge.
If your hospital stay continues through the following month, the account is closed at the end of each month. The bill for the month is delivered to your room on or after the 10th of the following month. Please pay the bill within one week of your receipt at the cashier on the 1st floor.
※Credit card and cash payments are accepted. Credit card payment is recommended for inpatients.
※An additional bank fee may be charged if you wire the payment to the designated hospital bank account.

(6) Support person

  • As a general rule, you do not need a support person accompanying you during your hospital stay.
  • Please inform the nurse if you wish to have a support person during your hospital stay.
  • A set of bedding for a support person is available for rent at ¥330 per day (including tax).

② Guidelines for visitors

Visitors are asked to observe the following guidelines:
  • To protect patients’ privacy, specific patient information may be provided only at the Security and Disaster Prevention Center.
  • Additional restrictions apply when visiting a patient in the hospital ward and ICU & CCU within the Emergency Medical Center, such as the length of a visit and the number of visitors allowed. Please ask the nurse for details.
  • Patients are asked to meet visitors in the common area as much as possible.
  • Visitation with accompanying small children is restricted in principle.
  • Visitors are asked to refrain from coming to the Medical Center when having symptoms such as a fever, cough, or feeling ill.
  • No visitation by a large group of people.
  • In principle, doctors can meet a patients’ family members on weekdays before 5:00 pm to explain about tests and surgical procedures.
  • Hospital staff do not take phone calls or phone messages for patients.
  • The hospital staff is prohibited from accepting money or gifts. Thank you for your understanding.
  • No live flowers are allowed into the hospital room.
