(1) Issuance of medical records
① Making a request
Please fill out a request form at the Medical Record counter (Counter No. 2 on the 1st floor).
② Fee
Fees vary depending on the type of document being issued. Please ask for details at the counter.

③ Picking up the requested document
The hospital staff will contact you by postal mail or phone when the document is ready for pick-up.
Please bring the document exchange ticket to the Medical Record counter (Counter No. 2 on the 1st floor).
The document is handed to you after the fee is paid in full.

(2) Eligibility Certificate for Ceiling Amount Application (GENDOGAKU NINTEI-SHŌ)
If you present the Eligibility Certificate for Ceiling Amount Application, you do not have to pay your copayment at the cashier after exceeding your ceiling amount. To apply for the Eligibility Certificate, please contact the city hall having jurisdiction over your residence or your employer’s health insurance provider.
